Documents required:
-Picture ID for everyone in the household 18 or older
-Social Security Cards for all family members or a legal document with the SS# on it
-Dependents birth certificate or 1 of the following: passport, DCF record reflecting name and DOB, official school printout record indicating DOB and legal guardian, or official immunization records with DOB and parents name
-Receipts for payments of unexpected expenses
-Current lease
-Landlord phone and fax number
-Original current detailed utility bills (NOT the late notices)
-Receipts for your payments for utility bills
-Bills for all monthly household expenses
-Current income for past 8 weeks for all household members (food stamps, Social Security, Paystubs, unemployment, child support, Workman's comp, any other type of income)
-If you are claiming a reduction in work hours a letter from your job stating the date starting, rate of pay, number of hours guaranteed per week.
-If bills are electronic, you must print them out and bring them
