Provides mental health programs on a referral basis. Services include assessments, crisis intervention, individual/group therapy, rehabilitation services, a dual diagnosis program, case management, prescription medication and monitoring, neuropsychological and psychological testing and outreach prevention. This site also has WIC services in Suite 210 M-F, 8am-5pm (call 408-918-5500). Accepts Medicare, Medi-Cal, and other State programs. Sliding scale available for uninsured. Hours: Mon-Fri 8 am - 6 pm
